By making this payment, I acknowledge that I have completed my ESA evaluation and assessment by a licensed mental health provider, who I see regularly for mental related matters who has added an ESA to my treatment. I acknowledge that the ESA letter is good for 12 months and should be renewed thereafter by a licensed mental health professional.
